Do you know the reason for referral?
WHO TOLD WHOM WHAT?
- The reason for referral should be clearly stated and justify the Medical Necessity for treatment. Ask yourself: WHAT UNDERLYING IMPAIRMENTS RELATED TO THE FUNCTIONAL DEFICITS NOTED REQUIRE THE SKILLS OF A THERAPIST TO RETURN THE RESIDENT TO HIS/HER PRIOR LEVEL OF FUNCTION?
- Reference nursing, family, physician as the source of referral rather than just from an admit screen
- Document if the patient is referred after a fall, or due to weight loss, or a change on the MDS
- If the patient’s needs were identified during a routine screening, cue nursing to document these needs/changes prior to obtaining the evaluation order
- Document what problems the patient is at risk for without skilled intervention
EXAMPLE: Patient referred to [discipline] skilled services secondary to [reason for hospitalization/decline] resulting in decline of [deficits]. Patient is at risk for [problem areas] without skilled intervention.
“Patient referred to PT skilled services secondary to falling at home causing a left hip fracture with resulting decline in gait, transfers and all functional mobility. Patient is at risk for further decline in mobility and fall risk without skilled intervention.”

Flexibility and leadership help Michigan therapist
Ashok Kumar Gunda was born in Hyderabad, India. He received his Bachelor’s degree in Physical Therapy from Bangalore, India in 2002. He practiced as a physical therapist in India from 2002-
2006 and then immigrated to the USA in 2006. He went to school for his Master’s in Rehabilitation Science at Concordia University, Wisconsin and graduated in December 2008.
Ashok started his career in the US as a physical therapist in Indiana and then decided to move to Michigan. He joined LaVie Rehab in 2010 and started working at Ovid Healthcare. Ashok and his wife Srujana recently became first-time parents of a beautiful baby girl who is now 3 months old. They named their daughter Aashritha.
Ashok’s goals are focused on improving his skills in therapy and mastering the management of therapy programs to provide exceptional services to his patients.
He is being recognized as a clinical MVP for his leadership skills, flexibility and being a team player. He is willing to help wherever he is needed and is friendly and helpful to his co-workers. He has been shuttling between two facilities in Michigan and covering as DOR in Pine River since January 2011 very effectively.
Ashok is very humble by nature and when he found out about the clinical MVP award, he said, “My family and I felt a sense of pride for being recognized for simply doing my job, helping my colleagues and the company.”

Pet's name: Moe Sterken |
Moe is a 5-year-old Eskimo Spitz. He had his summer hair cut done especially for this photo op. Luxuriating in the center of attention is Moe’s favorite daily activity. He participates in sports events such as chasing birds in his back yard, irritating the cat, sleeping and shedding. Moe always has a positive attitude and a ready smile!
